MNSI Annual Report 2025/26 published
This years annual report highlights work carried out between April 2025 and March 2026 to improve maternity and newborn safety in England.
During the year, MNSI completed 618 independent safety investigations and made more than 1,300 safety recommendations.
The report also showcases how MNSI has shared learning and developed its approach during 2025/26, including:
- launch of MNSI’s two-year programme strategy (2025 to 2027),
- development of the SPARC coding framework for deeper thematic analysis,
- expansion of the COMPASS pilot to 29 NHS trusts to capture how culture affects safety
- continued use of health equity tools (HEART and HEWS) in all new investigations.
MNSI will continue to build on this work in 2026/27.
