MNSI Annual Report 2024/25 published
MNSI’s Annual Report 2024/25, highlights work carried out between April 2024 and March 2025 to improve maternity and newborn safety in England.
Between April 2024 and March 2025, MNSI completed 613 independent safety investigations and made 1,122 safety recommendations to 126 healthcare organisations.
The report also showcases how MNSI has shared learning and developed its approach during 2024/25, including:
- a clearer reporting style designed for families and trusts,
- new safety prompts to help trusts reflect on local practice,
- piloting the COMPASS tool to capture cultural issues affecting safety, and
- embedding health equity tools (HEART and HEWS) in all new investigations.
MNSI will build on this work in 2025/26, expanding its thematic learning through new coding systems and AI-supported analysis, while continuing to share learning nationally and internationally.
